Midway MX Minor Release Forms

This minor release form must be used for ALL open practice. It must be signed by parent and notarized whenever parent or gaurdian will not be attending practice with the minor. Minor release form is also available at the track for minors attending with parent. There is also a yearly minor release card application available at the track.  Please click the link below for the practice minor release form.
